PCMH - Medicare Applications

This Medicare Model of Care session will educate participants on Medicare applications of patient-centered medical homes and will reinforce PCMH practices as well as how they apply to seniors on Medicare.

Learning Objectives:

• Reinforce core principles of the patient-centered medical home (PCMH) model.
• Describe how PCMH elements support quality, continuity, and outcomes for Medicare patients.
• Identify ways to adapt PCMH workflows to better serve seniors with complex needs.
