This group is for health center staff that are involved in the Community Health Center's Patient-Centered Medical Home (PCMH) work and submission. The purpose of the group is to provide support, discuss ideas, and share resources with each other. Discussion topics change monthly and requests for specific topics can be made to Dana Henderson.
