Strategies and Best Practices in the Management of Chronic Conditions. This series of three webinars and three TA sessions for clinical care leaders, managers, and staff. It is designed to support the delivery of enhanced care management practices for patients with complex health concerns with the aim of improving the quality of life and health outcomes for your patients.

Webinar #1 - Care Management Strategies from Basic to Advanced
Our first kickoff meeting will be an opportunity to review the fundamentals of Care Management, identify evidence-based components of successful care management models, and discuss how care management is a key driver in achieving improved health outcomes for patients with complex care needs.

Presenter: Brittany LaBarreare, MBA, RN Senior Consultant, HMA
Brittany LaBarreare is a healthcare delivery systems leader specializing in clinical operations, quality assurance and improvement, accreditation readiness and response, care management, and population health development and implementation.

She has years of experience in direct patient care and healthcare administration in ambulatory, longterm, and in-patient care settings. Throughout her career, Ms. LaBarreare has led with a hands-on approach to the daily operations and clinical strategies of these organizations and has developed successful programs and initiatives to improve delivery of care for those being served.
Prior to joining HMA, she worked for a large federally qualified health center in Texas, where she managed the care management department and population health programs. In this role, Ms. LaBarreare was responsible for designing, implementing, and tracking success for an integrated behavioral health program as a part of the 1115 Waiver Delivery System Reform Incentive Payment program. She also implemented a comprehensive care management program designed to improve the quality of life and clinical outcomes for high-risk and clinically complex patients. Ms. LaBarreare was responsible for employee relations, staff training, and ongoing quality/process improvement.

At the Texas Department of Aging and Disability Services, she developed training curriculums for intellectual and developmental disability (IDD) Medicaid waiver programs, as well as the policy, procedures, and training curricula for Texas Health Steps IDD transition population. Ms. LaBarreare collaborated with other state agencies in preparation for the managed care implementation for state recipients. She evaluated quality and cost effectiveness of services and assessed compliance with Medicaid program service requirements and state rules and regulations.

Ms. LaBarreare previously had roles at the Austin State Hospital and the Austin State Supported Living Center, where she provided direct nursing care and administrative oversight to individuals with IDD and serious mental illnesses.
