TPCA's 2021 Chronic Disease and COVID-19 Summit
13% of Tennessee's population has diabetes and 39% have hypertension. Navigating chronic illnesses during the COVID-19 pandemic requires public health and medical professionals to be innovative in their education of and engagement with their patients. Early detection, referral and care for these illnesses are integral to clinical measure outcomes and quality improvement for community health centers.
Intended Audience
Health Center Leadership, Providers, Quality Teams, Integrated Care Teams, Behavioral Health Providers, Pharmacists, and other health center staff as appropriate.
