TPCA's 2021 Chronic Disease and COVID-19 Summit
13% of Tennessee's population has diabetes and 39% have hypertension. Navigating chronic illnesses during the COVID-19 pandemic requires public health and medical professionals to be innovative in their education of and engagement with their patients. Early detection, referral and care for these illnesses are integral to clinical measure outcomes and quality improvement for community health centers.

Intended Audience
Health Center Leadership, Providers, Quality Teams, Integrated Care Teams, Behavioral Health Providers, Pharmacists, and other health center staff as appropriate.

Presenters: Dr. Elizabeth Wolff (Bio) will moderate a panel of health center leaders for TN FQHCs

 
In this panel presentation, we will discuss how health centers creatively care for their patients, lessons learned, and what improvements they will continue after the pandemic.  At the end of this session, participants will be able to:
  • Outline ways that they can incorporate telemedicine into the ongoing management of chronic conditions
  • Describe how remote patient monitoring can improve management of chronic conditions
  • List key skills and actions that clinical leaders can utilize in driving change

Session THREE (Emergency Preparedness) | REGISTER

Presenter: Melissa Corrado, MBA, Health Management Associates (Bio)

The final session of the chronic disease summit will focus on best practices and lessons learned for preparing community health centers for public health emergencies. The speaker will discuss vaccinations, general guidance on preparing for emergencies and how health centers can ready themselves for public health emergencies.
