TPCA's 2021 Chronic Disease and COVID-19 Summit
13% of Tennessee's population has diabetes and 39% have hypertension. Navigating chronic illnesses during the COVID-19 pandemic requires public health and medical professionals to be innovative in their education of and engagement with their patients. Early detection, referral and care for these illnesses are integral to clinical measure outcomes and quality improvement for community health centers.

Intended Audience
Health Center Leadership, Providers, Quality Teams, Integrated Care Teams, Behavioral Health Providers, Pharmacists, and other health center staff as appropriate.

Session ONE (National Perspective)
Thursday, July 15, 2021 from 1 - 2:30 pm Central

Presenter: Lisa Harrison, MS, MHS, PA-C, Health Management Associates (Bio)
This session will explore all the lessons learned of using technology to provide care to patients throughout a global pandemic. It will also cover trends and best practices in patient care using technology, and lessons learned throughout the COVID-19 pandemic with a focus on hypertension and diabetes. 

Learning Objectives:
  • Assess current evidence-based practices in chronic illness care during COVID-19 pandemic from a national perspective.
  • Discuss lessons learned on providing care during a public health crisis.
  • Form recommendations for continuing care with patients through the use of technology.
Session TWO (Tennessee Perspective) | REGISTER
Thursday, July 22, 2021 from 1 - 2:30 pm Central
Presenters: Dr. Elizabeth Wolff (Bio) will moderate a panel of health center leaders for TN FQHCs

Though much of the world was navigating the COVID-19 pandemic, populations with chronic illnesses such as diabetes and hypertension remained in need of care. Technology presented an unique opportunity to keep patients engaged and manage their care. This moderated panel will discuss the lessons learned and innovations of providing care through technology.

Learning Objectives:
  • Assess current evidence-based practices in chronic illness care during COVID-19 pandemic from a statewide perspective.
  • ?Hear multiple perspectives on providing care during a pandemic.
  • Discuss innovations and lessons learned on providing care during a global public health crisis.
  • Form recommendations for continuing care with patients through the use of technology.
     
Session THREE (Emergency Preparedness) | REGISTER
Thursday, July 29, 2021 from 1 - 2:30 pm Central
Presenter: Melissa Corrado, MBA, Health Management Associates (Bio)

The final session of the chronic disease summit will focus on best practices and lessons learned for preparing community health centers for public health emergencies. The speaker will discuss vaccinations, general guidance on preparing for emergencies and how health centers can ready themselves for public health emergencies.

  • Discuss best practices in emergency preparedness during public health and natural disaster emergencies.
  • List general guidance on preparing for emergencies.
