Join us for a full day educational session led by the National Health Care for the Homeless Council. Health center staff will leave fully prepared to use social risk factor data and integrate social risk factor care coordination into daily workflows.

  • What SDOH questions to ask and how to ask in a trauma-informed way
  • Highlight PRAPARE and other SDOH screening tools
  • Identify who is responsible for SDOH screening and how to include in it the workflow
