Tennessee Health Care Innovation Initiative:
Primary Care Transformation Webinar

6:00 - 7:00 p.m. Central Time

Primary care transformation focuses on the role of the primary care provider in promoting the delivery of preventive services and managing chronic illnesses over time. The initiative is developing an aligned model for Patient Centered Medical Homes (PCMH), Tennessee Health Link for TennCare members with the highest behavioral health needs as well as a shared care coordination tool that includes hospital and Emergency Department admission, discharge, and transfer alerts for attributed providers. The presentation is a review of TennCare’s role in Health Care System and Payment Transformation and how it will impact primary care providers. This involves a discussion of both Episodes of Care and Patient Centered Medical Homes. The goal is that each attendee understands the direction health care reform is moving in the state and how it will impact their practice.
