
Located in the southern half of rural west Tennessee, HardemanCounty Community Health Center (HCCHC) provides quality, affordableprimary health care to the citizens of the area. Located in Bolivar, thecounty seat, a town of approximately 7,000 we have a prime location next door to the community hospital. We are completing construction of a new5,000 square foot facility for the center across from our currentlocation. It will be finished in December.
The center enjoys much community support. We ahve been open since March 1993, and have experienced phernomenal growth each year. Infiscal year 1996, we have over 86,000 patient encounters. About 50% ofour patients are TennCare, 10% are Medicare, 23% are privateinsurance,and 17% are on a sliding fee scale based on income and familysize.
The center is a non-profit corporaiton, chartered in Tennesseeand governed by a board of directors. The board consist of communityresidents representing local business, government, health care, andvarious other fields. At least 51% of the board are users of the center.
The center is managed administratively by an executive director,and medically by a clinical director. Other administrative staff includea fiscal officer, medical receptionist, billing clerk, and file clerk. Other clinical staff include a physician, two LPNs, and a lab assistant. With a mission to provide quality accessible, affordable health care toeveryone, the center maintains a customer-oriented approach to management. Teamwork is encouraged and practiced.
HCCHC is one of 17 federally funded (330) community healthcenter in Tennessee, and one of 850 in the United States.
