Simple Tools for using i2iTracks for Care Managers and Care Coordinators

Who should attend this training? Care Managers and Care Coordinators

You will receive a simplified overview of i2iTracks functionality specifically related to care management and care coordination. This training is designed to prepare staff to use tools that have already been created in i2i Tracks, such as morning huddle reports, patient visit summaries, and patient searches. This training is designed to help care management and care coordination staff incorporate i2iTracks Follow-ups into their daily workflows.

i2iTracks 101: Back to Basics

Who should attend this training? Health center staff who are new i2iTracks users. This course can also be used as a refresher course for those with limited i2iTracks experience.

You will receive hands-on training on managing patients with chronic conditions, creating patient searches and basic reporting, basic EHR mapping and data validation, and integration of pre-visit planning for your morning huddle.

Advanced i2iTracks Day 1
i2iTracks 301: Population Health Intelligence through Analytics (iPHA reporting) (1.5 days)

Who should attend this training? Health center staff with moderate experience using i2iTracks and developing patient searches.
 
Population Health Intelligence through Analytics features step-by-step instructions for creating and optimizing reports, dashboards and advanced patient searches in i2iTracks. We’ll have extensive hands-on training with real-world exercises designed to help you meet the following goals:
  1. Improve quality of care by creating tools to measure progress, define benchmarks and answer questions about patient populations
  2. Create provider report cards that compare performance by provider for a variety of clinical measures
  3. Generate custom reports for health center boards, QI committees, PCMH, federal or foundation grants, and other program requirements. 

 
November 17
Advanced i2iTracks Day 2
 
Who should attend this training? Health center staff with moderate experience using i2iTracks and developing patient searches.
 
We will continue with Day 2 of the i2iTracks 301: iPHA report training. After we complete iPHA reporting we will review how to map and generate UDS Tables 6a, 6b, and 7 using both the hard-coded and custom iPHA UDS reports.
 
In the last section, we will review the fundamentals of Data Stewardship and preview the Data Element Manager, which will allow for improved mapping functionality across both iPHA reports and the i2iTracks profile item library.
